Who this applies to

A person under 65 with disability who is living in (or being discharged from) an aged care facility, and who is eligible for the NDIS, has the right to transition to NDIS-funded disability supports in an appropriate home — not an aged care setting. The NDIS is specifically designed to be a better fit for their support needs and life goals.

For many years, younger Australians with disability ended up in residential aged care because there were no alternatives available, or because the system defaulted to the most accessible bed. The NDIS changed the funding landscape — but the process of actually moving from aged care to NDIS-funded SIL is complex and requires careful coordination.

We understand the clinical, administrative, and emotional dimensions of helping someone leave a setting that wasn't designed for them and move into a home that is. The process takes longer than a standard hospital discharge — the right support matters more than the fastest available option.

NDIS eligibility support

Finding a compatible SIL home that meets the participant's clinical needs and reflects their preferences and goals — not just what's available.

How it works

The aged care to NDIS
transition process

This is one of the more complex transitions in the NDIS system. It involves multiple organisations, often some NDIS plan work, and a careful matching process. We are honest about the timelines involved.

Initial assessment

We talk with the participant, their family, and their support coordinator to understand the current situation — how long they've been in aged care, what their clinical needs are, and what they're hoping a new home will look like.

NDIS plan review

We review the existing NDIS plan (or support the application if there isn't one). For aged care transitions, the plan typically needs to include SIL funding, and we advise the coordinator on what evidence and reports are likely to be needed.

Home identification & matching

We identify SIL homes that are appropriate for the participant's needs, cultural background, interests, and goals. We don't match on need alone — the person who will live in the home matters, not just the support ratio.

Facility coordination

We liaise with the aged care facility to coordinate the handover of medical records, medication management, and clinical documentation — ensuring nothing falls through the gap during the transition.

Transition support

The move itself is supported carefully. Where possible, we arrange overlap — so the participant has time to settle into the new environment before aged care support formally ends.

Post-transition review

We review the first weeks in the new home carefully. For some participants, especially those who have been in institutional care for years, the adjustment takes time — and we build that expectation into the plan from the start.

Common questions

Questions about aged care
to NDIS transitions

Can a person under 65 in aged care access the NDIS?

Yes — if they are under 65 and have a permanent disability that meets the NDIS access criteria, they are likely eligible for the NDIS. Being in aged care does not exclude someone from NDIS eligibility. In fact, the NDIS was specifically designed so younger people with disability would not have to rely on aged care settings.

How long does an aged care to NDIS transition take?

These transitions typically take longer than a standard hospital discharge. Factors include: whether there's already an active NDIS plan with SIL funding, the availability of compatible SIL homes, and the complexity of the participant's clinical needs. MMT Care is transparent about this timeline — we don't set expectations we can't meet.

What if the person doesn't have an NDIS plan yet?

The NDIS access and planning process needs to happen before a SIL placement can be confirmed. MMT Care can advise on the evidence and reports likely to be needed, but the support coordinator or LAC manages the NDIS application. We'll work alongside that process so we're ready to match as soon as funding is in place.

Does MMT Care manage in-home SIL for aged care transitions?

Yes — for some participants, in-home SIL is a better fit than a group home, especially for people who have been institutionalised for a long time and need a quieter, more personal environment to adjust. We'll discuss both options and recommend what we genuinely think will work best for the specific person.
