Yes. We work with Gold Coast University Hospital (GCUH) and Gold Coast Health discharge planning teams to coordinate transitions into SIL. Because NDIS SIL approval requires a detailed Roster of Care and takes time, we recommend beginning the planning conversation as early as possible so the process can run alongside the clinical discharge timeline. A registered NDIS provider has been independently audited against the NDIS Practice Standards and approved by the NDIS Quality and Safeguards Commission. Participants using NDIA-managed funding can generally only engage registered providers; plan-managed and self-managed participants have more flexibility. MMT Care primarily delivers SIL and in-home SIL across the Gold Coast. Specialist Disability Accommodation (SDA) is a separate NDIS funding category for participants with extreme functional impairment or very high support needs — only around 6% of participants are eligible.
